Woven Name Tags
Name tags are ideal for identifying personal belongings especially for school children's uniforms and laundry in senior citizens' nursing homes
To order your name tags now:
- Print this order form
- Write the name you want to order in the space provided next to 'name required'. (One name per order. Length must not exceed 30 characters and spaces)
- Put an 'x' next to the quantity you require, 3 dozen, 6 dozen or 12 dozen
- If you prefer to iron on rather than sew-on the tags, put an 'x' next to 'iron-on'
- If you prefer to receive the tags by post, put an 'x' next to 'post' and write your address in the space provided
- Choose the colour in which you want the name to be woven by circling black, blue, red or green (background will be white)
- Add up the cost and send a cheque together with your order to Special Delivery Ltd., 75, Old Railway Track, St. Venera SVR 9016 or hand in the order form at the shop (Allow approximately 2 weeks longer at peak times such as September)
